Assisted Living Residence at Golden Years

Information

The Assisted Living Residence at Golden Years is a Wisconsin licensed Community Based Residential Facility (CBRF) for the frail elderly. We provide personal care and supportive services for 27 residents.
Residents are encouraged to function as independently as possible. However, when a little extra help is needed, our well-trained staff are there to offer assistance, comfort and support.

Room Types

Services

  • Wireless nurse call system
  • Medication monitoring (Pharmaceuticals are billed to the resident by the pharmacy provider)
  • 24/7 Resident assistance with nurse manager assessments
  • Full meal service plus snacks
  • Daily activities and special programs
  • Physical therapy, Occupational therapy and Speech Language pathology*
  • Personal and flat laundry
  • Housekeeping
  • On site beautician (fees for service are paid to provider by resident)

    * Fees for these services are billed to Medicare or other insurance carrier, if applicable.

Staffing

The Assisted Living Management team consists of the nurse manager, assistant manager, nursing assistants, activity director and dietary, housekeeping, laundry and maintenance personnel.

Financial Considerations

Waiting List

A $500 waiting list deposit and application are required to secure a position on the Assisted Living Waiting List. The waiting list deposit is applied to the first monthly statement or is fully refunded if the resident’s name is withdrawn from the list.

Monthly Statement

In addition to the resident’s room, the following services are included in the basic monthly room rate:

  • Basic assistance and supervision, round-the-clock
  • Medication administration and monitoringFull meal and snack service
  • Social Service assistance and intervention
  • Life enrichment activities and programs
  • All utilities except cable and telephone service
  • Daily housekeeping service
  • Facility maintenance upkeep and refuse removal

Additional fees are incurred when a resident’s need exceed basic care or when additional supplies or services are provided. A listing of extra care fees is available and is given on request and at the time of admission.

Application Process

Individuals interested in the Assisted Living Residence are encouraged to tour our home, meet with the manager to discuss issues and concerns, complete an application for residency, make a $500 deposit and go on the waiting list. The $500 deposit is applied to the first monthly statement or is refunded to the individual if their name is withdrawn from the Waiting List. Rooms are filled chronologically from the waiting list with priority given to residents within Golden Years.
